Data on occupational injuries to police officers for a period from 1/1/2009 - 5/31/2015. Specifically we request the following information from occupational injury reports: Employee unit # Incident location Incident date What employee was doing/how accident occurred Nature of injury Name of immediate supervisor Supervisor serial # For the following fields, we would not need exact details re: treatment, but if possible we would like to get an indication of the kind of treatment received. For example a "y" if the field is filled in and an "n" if the field is empty would be great: First aid Primary treating physician Other doctor Hospital Thank you -
